Form Link for Sheets™
Form Link for Sheets™ is a Google Workspace™ add-on that fills PDF forms from your Google Sheets™ data — every row becomes a completed PDF, saved straight to your Drive, and your spreadsheet data never leaves Google.
What Form Link for Sheets does
Form Link for Sheets is a Google Workspace™ add-on for filling out PDF forms from spreadsheet data. You open it in a Google Sheets™ spreadsheet, choose a fillable PDF, and it produces one completed PDF for every row — then saves them to a folder in your Google Drive™ that you name, and writes a link to each finished file back into your sheet.
It is built for anyone who fills in the same form over and over: consent forms, intake paperwork, registrations, waivers, permission slips. Instead of retyping the same answers into the same PDF dozens of times, you keep the information in rows and let the add-on do the filling.
To do that, the add-on reads the spreadsheet you have open, reads the PDF template you pick, and creates the PDFs and the folder they go in. It does not read anything else in your Drive, and it sends nothing to any server outside Google. Full detail is in the Privacy Policy.
How it works
- Pick a template. Choose any fillable PDF from your Drive using the Google Picker.
- Map your columns. Match spreadsheet columns to the PDF's fields in the sidebar.
- Generate. Form Link fills the form for each row and saves the PDFs to a folder in your Drive that you name, linking each one back into your sheet.
Your spreadsheet data stays in Google
PDF generation runs in-process in Google Apps Script — no spreadsheet data, template, or generated document is ever sent to a non-Google server. The add-on only touches the spreadsheet you have open and the files you pick or it creates — never the rest of your Drive.
Two things do leave Google, and only these: when you open the sidebar, the add-on checks with form-link.ai that your account is active, and after a run it reports how many PDFs were generated. Those requests carry your email address, your Workspace domain if you have one, and counts — never cell values, form field contents, or PDF contents.

What it can access
Form Link for Sheets requests only minimal, per-file Google permissions:
- The spreadsheet you have open — to read the values you map and write a link to the generated PDF back into the sheet. It cannot see your other spreadsheets.
- Only the files you pick or it creates — the template you select via the Google Picker, and the PDFs it generates. It has no access to the rest of your Drive.
No full-Drive access, no Gmail, no whole-spreadsheet access. Form Link adheres to Google's API Services User Data Policy, including the Limited Use requirements.
Frequently asked questions
It’s a Google Workspace add-on that turns Google Sheets rows into completed PDF forms — every row becomes a filled PDF, saved straight to your Drive.
Your spreadsheet data, your templates, and the PDFs it generates never do — all of that is processed inside Google Apps Script and saved to your own Drive. The add-on does contact form-link.ai, which we run, for two things: to check that your Form Link account is active, and to count how many PDFs you generate. That sends your email address and counts only — never cell values, form field contents, or PDF contents. See the Privacy Policy for the full list.
Only minimal, per-file Google permissions: access to the spreadsheet you run it on and the ability to save the generated PDFs to your Drive.
No — the output is standard PDF files in your Drive. Share them like any other file.
